MediNail Indemnity Liability Waver

If you hold your training in a hotel or other facility you may be asked to provide insurance to protect the owner of the property or if an injury occurs during the training unless it is your salon, or you have an arrangement with a school.
•	You do assume full liability for your own hands-on training company and your instruction as the only part MediNail is responsible for is the online training component of your class and for collecting the online component only.
•	I attest I will collect, protect financial information or each student and be responsible for all monies collected from each student for the hands-on ANT training I am teaching
•	I am providing an autoclave and an electric file with file bits or manual files that are safe and has been evaluated and checked for functionality, safety and will protect my students.
•	I will properly dispose of any sharp nippers, salon tools or files that are made to be disposable.
•	You attest that you have completed all the necessary training to become an ANT Instructor, your ANT Instructors certificate is not expired 3 years from being issued and that you will follow your state and the states scope of practice you hold the ANT training in.
•	You will not be teaching sharp debridement and you will not have and credo blade or cheese grate skin removal tool of any type 
•	Students can get injured with sharp nippers
•	Students can get injured from falls if their feet are wet or covered with slick foams, detergents, soap or creams
•	Student can get burned from hot items in the autoclave and from venting the autoclave and I will provide a hot mitten holder to protect them.
•	Students can be exposed to COVID or other airborne illness, fungal, bacterial or viral skin infections and it is suggested a waiver be signed to acknowledge that fact.
•	Be aware of any issues with weather or the facility that would make having or operating a class unsafe.
•	If you are concerned you should draft a student liability waiver for them to sign that they hold you and the facility harmless if they get injured
•	By completing the ANT Instructors course and providing future or actual training to your private individual ANT Hands on students, you agree to hold MediNail, Inc. harmless from any ANT instructional activity that you oversee and are in charge of preventing any injuries or damage that may occur, monies collected for your individual companies ANT Hands-on Training, equipment necessary for your ANT instructional course and any facility contracts or agreements you arrange.

I understand and accept the above standards to instruct my class and agree to protect and provide proper liability insurance, waivers or policies to make my class and students safe. I acknowledge MediNail Inc is not arranging or providing the ANT class, venue or insurance for the ANT Instructor and the ANT Instructor is conducting and is assume all risks and liabilities for the onsite Hands-On Instructional education.
